Who we are

This Privacy Policy describes how Salon Bales LLC d/b/a Bales Health Systems (“BHS,” “we,” “us,” or “our”) collects, uses, and shares information in connection with www.baleshealth.com and related business operations.

BHS is a healthcare management / management services organization (MSO). Our headquarters are at 2108 N St Ste N, Sacramento, CA 95816. Use the Request a quote form for contact.

Role separation — clinical care

BHS does not provide clinical care and is not a covered healthcare provider under this Policy. Clinical care is furnished by independently licensed practices (for example, Salon Bales PC) and partner clinical networks. Those entities maintain their own privacy notices for patient health information and medical records.

This Policy covers website, CRM, marketing, partner, and operational business data processed by BHS as a management company — not clinical records held by licensed practices. We do not present this notice as a HIPAA Notice of Privacy Practices for a covered entity.

Information we collect

Depending on how you interact with us, we may collect:

• Contact and business details you submit (name, email, organization, role, inquiry content) via forms or email.
• Operational / CRM data related to practices, partners, employers, and program distribution.
• Technical data such as IP address, browser type, device information, pages visited, and approximate location derived from IP, collected via cookies or similar technologies.
• Communications preferences if you opt in to email or SMS (see our SMS Terms).

How we use information

We use information to:

• Respond to inquiries and manage business relationships.
• Operate, secure, and improve the website and internal systems.
• Support MSO services, care-ops tooling, CRM, and program distribution for practices and partners.

Categories of personal information we may collect in the prior 12 months include identifiers (name, email, IP), commercial / professional information related to business inquiries, and internet activity on our site. Sources include you directly and automated collection. Business purposes include those listed under “How we use information.”

Children

Our website is directed to businesses and adults. We do not knowingly collect personal information from children under 13.

Changes

We may update this Policy from time to time. The effective date at the top will change when we do. Continued use of the site after updates constitutes acceptance of the revised Policy where permitted by law.
